Patient Forms

We strongly recommend new patients print off forms and fill them out at home before arriving to their first visit. Rather than trying to fill out forms at the office, you can walk in prepared and expedite your time in the waiting room.

While we understand no one enjoys completing forms, relevant information about you, your symptoms, and overall medical history are a necessary part of providing the best care and creating a comprehensive treatment plan.

What Else Should I Bring?

Additional things we recommend bringing to your initial visit:

  • Insurance card, if you have one.
  • The shoes you wear to work, and the shoes worn for exercise. Observing the tread wear can sometimes reveal information about the sources of your foot pain.
  • Any current medications, current conditions, prior surgeries/hospitalizations, test results, medical records, etc., that weren’t listed on your new patient form or sent to us by your referring physician. The more information we have, the better able we are to provide safe, effective, and personalized care.
  • A list of questions to ask. As you go through an appointment, your mind may be pulled in many directions, and you may forget to ask a question you had intended to ask. Writing your questions out beforehand helps ensure that we address all your concerns.
